Adult in Camp State Compliance form

Description: This form is required for all adults by the Texas Youth Camp Compliance Safety Act and must be completed to attend a Texas Youth Camp.

Annual BSA Health and Medical Record

Description: This form is required for all participants and volunteers, youth and adult. A current (within 12 months) health history (Parts A and B of form 680-001) for each camper, leader, and staff member is on file in camp during the camping period.

Camper: Scout registration form

Description: This registration form should be used for the following Cub Scout ranks: Tiger, Wolf, Bear, and Webelos. Please return this completed form to your Pack representative to be submitted to your district’s day camp registrar. A separate form must be completed for each participant. Incomplete forms will not be accepted.

Camper: Sibling registration form

Description: This registration form should be used for the following: Siblings of Scouts with parent volunteering 4 days during Day Camp. To be eligible to enroll and participate in the Scout Program siblings must be 7 years old or have completed first grade by the start of camp, and a parent must volunteer at Day Camp all four days.

Camper: Tot-Lot registration formUpdated 04/24/2013

Description: This registration form should be used for the following: children of full-time volunteers staying in Tot-Lot. YOU MUST BE PREAPPROVED TO USE THIS FORM. The Tot‐Lot is available only for children of full‐time volunteers at Tri‐District Cub Scout Day Camp. ALL CHILDREN MUST BE POTTY TRAINED. Please contact your district’s Day Camp registrar to determine if you qualify before submitting this form. Separate forms must be completed for each participant. Incomplete forms will not be accepted. Please return this completed form to your Pack representative to be submitted to your district’s Day Camp registrar. Reserved placement cannot be guaranteed after May 1, 2013.

Description: This registration form should be used for the following: Day Camp staff and volunteers over the age of 17 years. Separate forms must be completed for each participant. Incomplete forms will not be accepted.

Volunteer: Youth registration form

Description: This registration form should be used for the following: Day Camp staff and volunteers under the age of 18 years. Separate forms must be completed for each participant. Incomplete forms will not be accepted.
